Emergency Referral

In England approximately 40% of all emergency admissions that go into hospitals get there via urgent and emergency referrals (NHS England 2013). These patients are often admitted with insufficient information even though they were seen by a clinician or healthcare professional on referral. With having no end to end connectivity, letters often gets misplaced or lost and clinical staff are put at professional risk.
